LIABILITY WAIVER
I hereby affirm that I am, and/or my child(ren) named above in this form, are, voluntarily attending a class, event, or gathering with Playshire/Anima Speaks, LLC, Michael DePass, and Rebekah DePass. I am/my children are voluntarily participating in this experience entirely at my/their own risk. Events take place on the DePass’ home property in Greensboro, North Carolina, and I have been advised and am aware that there is a risk of injury. Playshire Events do not provide childcare, and I am always fully responsible for supervising my child and managing my child’s safety and well-being at all times during the event.
I have been advised that Playshire/Anima Speaks, LLC, Rebekah DePass, Michael DePass will not assume any liability for the transmission of any form of pathogens or any resulting illness that may occur from attendance or participation at this event. I have also been advised that Playshire Events are held outdoors in a natural setting in which there are steps, high dirt hills, dirt, tree stumps, trees, mud, rolling toys, tunnels, running water, steps, and all manner of potential pests and plants on this waterfront property, and that In full consideration of the risk of injury while participating in Playshire Events and for the right to participate in Playshire events, I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives, knowingly and voluntarily participate in this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my participation/my child(ren)'s participation in events, experiences, and play sessions by Rebekah DePass, Michael DePass, their affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, for any kind of risks related to or participating from Playshire Events, which may include, but are not limited to, physical or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability, economic or emotional loss, and death.
I acknowledge that I have carefully read this form and fully understand that it is a release of liability. I expressly agree to release and discharge Playshire/Anima Speaks, LLC, Rebekah DePass, and Michael DePass from any and all claims or causes of action, and I agree to voluntarily give up or waive any right that I may otherwise have to bring a legal action for personal injury or property damage.
